Midlink Magazine, a digital magazine for children
and educators, has a section on copyright
guidelines with links to permission request
templates.  I don't think the forms are exactly
what was mentioned in Amy's inquiry, but there
are links to other sites that may have information.

Does anyone have a good release form that they use that a parent/guardian
may
sign to legally give permission to place content created by his or her youth
on the Internet as well as identifying that youth on the Internet by name,
etc.

I am concerned about copyright/intellectual property rights issues, if they
apply.

If you could e-mail me the form or point to it on the web, I would
appreciate
it.
