Database Tracking Template

Hi,

I run an afterschool program and am wondering if anyone has a database
template that I could use for Microsoft Access (or Excel) where I can
easily track and organize students' attendance, payment of fees, address &
phone info, etc.  I would very much appreciate any help with this.  Thanks.
