[YL] Sample Staff and Executive Director Evaluation Forms?

I created it myself for internal use to the ETR Associates Training &
Program Development Department, so use it if it is useful to you.

I looked at this and it is great but who is the source that created it
and 
how is it benchmarked or how were the standards created.
If we adopted these standards usually a board or agency sanctioned
them.
Anyway I would like to use them but just need a source
